BAPTISM
As you are seeking the baptism of your child in the Roman Catholic Church, the following guidelines are presented to help you embark upon this journey of faith with Christ and his Church. Baptism, by actual reception or at least by desire, is necessary for salvation, and is the gateway to the reception of the other sacraments. (Code of Canon Law # 318)
BASIC GUIDELINES AND INFORMATION FOR INFANT BAPTISM
The baptism of infants should be administered without delay and with appropriate catechesis for the parents and godparents. (Code of Canon Law # 317)
It is the role of parents to provide the environment where a young disciple of Christ can grow. This will be most critical in the earliest years of life. This is where the Church needs parents to fulfill their role as evangelizers and Christian models—and where the Church needs to support parents in this great work.

In order that a child be baptized, it is necessary that the parents’ consent, and that there is reasonable hope that the child will be brought up in the Catholic Faith. They should be married in the church. They should furnish their family book

If the parents are from another parish, they should provide a letter of recommendation from their respective parish priest.
The parents are required to make confession prior to the date of baptism
GODPARENTS
In order for someone to be eligible as a godparent, they must be chosen by the parents,

They should have the ability and the intention to fulfill the role as godparents

Should be at least 16 years of age

Must be a confirmed Roman Catholic, who has already received the Most Holy Eucharist
-
Should be leading a life in harmony with the Catholic Faith (including Marriage)

Expected to be a good role model for the one being baptized

They should furnish their family book
